Step 2: Assessment & Carer Matching

We visit the home to meet your loved one, assess their stage of dementia, and understand their history, routines and preferences. We then carefully select carers with relevant dementia experience.

 

 

Step 3: Detailed Care Plan
We build a comprehensive care plan that captures personal history, daily routines, communication preferences and all care needs. It becomes the guide every carer follows.
Step 4: Care Begins
Your dedicated carer team begins providing consistent, familiar support. The PASS platform records all care in real time, giving families full visibility from anywhere.
Step 5: Ongoing Nurse Oversight

Our team monitors cognitive and physical changes, reviews the care plan regularly, and coordinates with the GP and any dementia specialists. Families receive proactive updates – not just crisis calls.
