Palliative & End of Life Care at Home in London

Compassionate end of life care at home - nurse-led, dignified, and deeply personal across North & Central London

About Our Palliative & End of Life Care

When a loved one is approaching the end of life, the priority is comfort, dignity and the ability to remain in the place they call home. At Golders Green Nursing, our palliative care service provides compassionate, expert support that enables people to spend their final weeks and months in familiar surroundings, surrounded by the people they love.

Our palliative care team brings together experienced carers and NMC-registered nurses to provide holistic support that addresses physical, emotional and practical needs – for the person in our care, and for their family.

We work closely with your GP, hospital consultants and any hospice or specialist palliative care teams involved, ensuring a fully coordinated approach and seamless communication at every stage.

What Our Palliative Care Includes

Our palliative care service covers all aspects of comfort and dignity at the end of life:

  • Personal care – gentle, dignified assistance with washing, dressing and hygiene
  • Pain and symptom management support, working alongside medical teams
  • Medication administration by NMC-registered nurses where required
  • Nutritional support and assistance with eating and drinking
  • Repositioning to prevent pressure sores
  • Emotional support, companionship and meaningful conversation
  • Practical household support to relieve family pressure
  • Overnight and 24/7 care availability

Support for the Whole Family

End of life care is not only about the person who is ill. Families carry an enormous emotional and practical burden, often while managing their own grief. Our team provides:

  • Regular communication and weekly reports so families always know what is happening
  • Respite support so family carers can rest
  • Practical guidance on what to expect at each stage
  • Liaison with hospice nurses, GPs, district nurses and specialists
  • 24/7 emergency out-of-hours contact for family members

Families frequently tell us that having Golders Green Nursing involved gave them the freedom to simply be present with their loved one – to hold a hand, share memories, and say goodbye without the burden of physical caring responsibilities.

How Our Palliative Care Service Works

We understand that palliative care often needs to be arranged quickly. We respond with speed and sensitivity.

Step 1: Urgent Consultation

Contact us as soon as possible. A senior team member – often a nurse – will speak with you that day to understand the situation and advise on the care options available. We can begin care rapidly when needed.

Step 2: Assessment & Care Plan

We visit to assess the person’s current condition, comfort needs and home environment. We create a care plan that prioritises comfort, dignity and the family’s wishes – including any advance care directives or DNR instructions.

Step 3: Coordinated Care Begins

Your dedicated care team begins working, coordinated with all other professionals involved. Our nurse maintains oversight and communicates regularly with the medical team.

Step 4: Flexible, Responsive Support

Palliative care needs change day to day. We adapt continuously – increasing hours, adding nursing visits, or arranging overnight care as required.

Step 5: Nurse Oversight Throughout

Your named nurse remains the central point of coordination – for the family, the GP, the hospice team and our carers. Nothing falls through the cracks.
